# Corrective and Preventive Action (CAPA) Management Template

## When to Use

- When a quality event requires investigation, corrective action, and an effectiveness check.
- When authorized quality personnel need traceable causes, action owners, and prevention evidence.

## When NOT to Use

- To close a recurring problem solely because an action was implemented, without checking effectiveness.
- Do not close an improvement solely because it was documented or assigned. See [Lessons archive](../../../../../docs/principles/anti-patterns.md#lessons-archive).

## Pairs Well With

- [Problem Management Process Template](../../../../../domains/delivery/industry-specializations/information-technology/service-management/problem_management_process_template.md).
- [Quality Management Review Template](../../../../../domains/delivery/industry-specializations/healthcare-pharmaceutical/quality/quality_management_review_template.md).

Selection context: Delivery domain; universal methodology; advanced complexity. Match the situations above to project phase, risk, team size, and industry using the [decision-engine context model](../../../../../meta/architecture-research/800-801-context-assessment-model.md).

## Applying this template

Treat filled examples, dates, numerical limits, sampling frequencies, and approval outcomes as illustrations. Before execution, replace them with the approved product/site requirements, scientific rationale, method references, responsible reviewer, and acceptance evidence. Record the applicable jurisdiction and current guidance edition; a completed worksheet alone does not demonstrate regulatory compliance.

| Requirement or limit | Applicable source / edition | Product-specific rationale | Approval / date | Execution evidence |
|---|---|---|---|---|
| [Requirement] | [Source] | [Rationale] | [Reviewer / date] | [Record] |

- Location/department where the issue occurred
- Products, processes, or systems affected
- How the issue was discovered (e.g., audit, complaint, routine monitoring)
- Initial containment actions taken, if applicable

Example:
> On 2023-06-15, during routine stability testing of Batch XYZ123 of Product A, an out-of-specification (OOS) result was observed for dissolution at the 12-month time point. The OOS result was confirmed through retesting. Initial review of manufacturing records did not identify any obvious deviations during production.
